Medical Marijuana Certifications

Step 1: DO YOU QUALIFY?

Under Arizona state law, you must have one of the following conditions to qualify for a Medical Marijuana Card:

  • Severe and Chronic Pain
  • Agitation of Alzheimer’s Disease
  • Amyotrophic Lateral Sclerosis (ALS)
  • Cancer
  • Crohn’s Disease
  • Glaucoma
  • Hepatitis C
  • HIV/AIDS
  • PTSD (must be currently under the care of a mental health professional)
  • Cachexia or Wasting Syndrome
  • Severe Nausea
  • Seizures
  • Severe and Persistent Muscle Spasms

Step 2: MEDICAL RECORDS

  • If you haven’t seen a physician within the past 12 months regarding one of the conditions stated above, please make an appointment to see a physician (Records from Chiropractor, Nurse Practitioner, or Physician Assistant will not be accepted).
  • Once the office visit is completed, request medical records from that appointment.
  • If you have been seen by a physician within the past 12 months, then please request medical records and bring to your appointment.
  • If you don’t have any medical records then you still may schedule an appointment. The physician will evaluate to see if you qualify for a medical marijuana card. If you do not qualify, you are only responsible for the $20 office visit fee.

Step 4: THE APPOINTMENT

  • You will pay for your appointment at the office.
  • The Medical Marijuana Card certification appointment usually takes about a 1/2 hour to complete. The physician will interview you and review/document your medical history, perform a thorough physical exam and recommend other treatments that may benefit your condition. All this will take place in a relaxing office setting, with full attention given to you.
  • The office will reach out to you 1-3 days prior to your appointment to confirm your appointment and make sure you have all necessary items for your appointment.

What Do You Need To Bring To Your Appointment?

  • You! Be ready to take a picture for your new card.
  • AZ driver’s license, state ID, or US Passport.
  • EBT Card (Must have your name on it) OR Award Letter for Nutritional Benefits from DES.
  • Medical Records
  • Current MMJ Card (if renewal application)
  • Payment for Fees (Cash is preferred method of payment. Additional fees for credit or debit cards):
    – Doctor Appointment Fee $100- $150
    – State Fee $150 (If EBT Card $75)

What Do You Need To Do After Appointment?

Nothing! Relax! Everything will be completed for you. Wait for your new card to arrive in the mail. Processing time is usually between 7-14 business days.
